Planning for the future is important, but often it is necessary to obtain assistance in marshaling the resources needed to execute the plan or to assure that help is obtained right now.
Whether or not a Life Care Plan is in place, our Care Management division can assist those who care for a disabled, chronically ill, or injured individual.
Our team consists of care management specialists, a psychologist, and an RN. We help our clients meet the challenges that are rarely addressed directly by healthcare providers, including:
- How to address quality of life issues
- How to obtain relevant entitlements
- How to analyze services and to select appropriate ones
- How to find supportive services for the identified client and their family
The Care for Living Program designs and customizes a care plan to meet the needs of individual clients, with health care issues being only the starting point. Our mission is to assist each person in obtaining the best possible quality of life given their circumstances.
Our participation can begin at any time – when a caregiver requires us to help currently or if administrators request our services when the primary caregivers are no longer on the scene and their wishes are to be executed faithfully.
